Consent to genetic testing services
This form is to relinquish Ana-Gen
Technologies, Inc. and it’s representatives from all
responsibility for the sample collection and any effects
or actions that the results from this test may have on any
of the individuals being tested.
Disclaimer:
By submitting the samples to Ana-Gen Technologies, Inc.,
you explicitly understand that the DNA testing provided by
Ana-Gen Technologies, Inc. is strictly for personal
reasons. You understand that the samples are collected
without a strict chain of custody; therefore, the result
may not be admissible in a court of law. By submitting the
samples, you agree to release Ana-Gen Technologies, Inc.
from any liability in connection with this test. By
submitting the samples to Ana-Gen Technologies, Inc. you
agree with all the terms below.
Terms
and Conditions
You hereby certify that you are
voluntarily furnishing these samples, appropriately
collected and labeled, to Ana-Gen Technologies, Inc. for
the purpose of genetic testing.
You clearly understand that the results from this
genetic testing may reveal information regarding your
relationship to others.
You also understand that these results may or may
not provide conclusive evidence to relationship analysis.
You further understand that these samples are being
collected and furnished to Ana-Gen Technologies, Inc. and
that AnaGen Technologies, Inc. reserves the right to send
these samples to outside laboratories for confidential
genetic analysis.
You also understand that the
turnaround time for the result is an estimate and NOT
guaranteed, and that most results are provided within
specified time after samples, with complete payment,
are received by Ana-Gen Technologies, Inc.
These genetic test results are being obtained for
your personal information only, and Ana-Gen Technologies,
Inc. does not guarantee that these results will be
admissible as evidence in a court of law.
However, you also understand that these results
could possibly be used as evidence in a court of law to
determine your relationship to others, and that you should
consult an attorney before you submit your samples for
determining your relationship to others.
You further understand that it is your
responsibility to determine if this type of testing is
legal in the state and/or country that you reside in, and
that some states may require that a court order be issued
or a doctor’s prescription is written for this type of
testing.
You further understand and agree
that while DNA Parentage/Identity Testing is highly
accurate, as with any testing, there is always a
possibility of error or omission. You acknowledge and
accept Ana-Gen Technologies’ limit of liability not to
exceed the cost of the test, and you agree to indemnify,
defend, and hold Ana-Gen Technologies, its employees, and
any person or entities collecting specimen harmless from
all further claims or damages. In addition, you agree to
contact Ana-Gen Technologies immediately, but in any event,
within 30 days from the date of the report, if you have
any questions or concerns about the testing process or the
outcome of the test.
You hereby release Ana-Gen
Technologies, Inc. and it’s representatives, from any
responsibility or injury that might result from collecting
specimens and from your actions that you may take after
receiving your test results.
You acknowledge that AnaGen Technologies, Inc. will
contact you with your paid results as you have requested
to be contacted on the accompanied Order Form.
You understand that these results could be based on
probabilities and that these probabilities may not provide
a definite conclusion.
You further certify that, that
AnaGen Technologies, Inc. has clearly indicated the cost
for each test. You understand the payment is due
regardless of the results obtained from the samples
provided. You
also understand that Ana-Gen Technologies, Inc. may require
more samples for testing, and will contact me either by
email, telephone, fax or letter carrier service in order
to obtain more samples.
